Also updates our coding style file.
Ancient CMake versions required upper-case commands. Later command
names became case-insensitive. Now the preferred style is lower-case.
Run the following shell code (with GNU compliant sed):

These are all timeout loops, which now use
std::chrono::steady_clock::now() to check for timeout events.

To avoid the proliferation of additional include directories and
multiple ways of including project-local headers, we now default to
moving all headers that are used across UHD into the uhdlib/
subdirectory.
Some #include statements were also reordered as they were modified for
closer compliance with the coding guidelines.
Internal cpp source files should now include files like this:
#include <uhdlib/rfnoc/ctrl_iface.hpp>

Now, use `do{...} while(0)` to pack multiple statements
into a single one that isn't fragile.
Currently
```
if(state_just_right)
OCTOCLOCK_SEND_AND_RECV(all,the,options);
```
would introduce annoying bugs.
Generally, there's no good reason this is a `#define` rather than a
C/C++ function.
The `OCTOCLOCK_PACKET_MATCHES` should probably be a C function, too,
or simply an overloaded `==` operator on to `const octoclock_packet_t&`.
